they asked ur height, weight, blood type i think, then your mother, father occupation. if not wrong also ur siblings and dunno what... a lot of health related and personal questions...

a lot of intrusive questions and is ask u to fill up in the form. and it's not even like second round or confirmation hire they buy insurance for u
