I'm more interested in the cause than the damage done
Train axle box and one bogie misaligned and then fell off. The train kept moving along the track and it continuously damaged other things on the rail.

Bolts and screws weaken or not secured properly.
Mechanical stress and fatigue failure on the connecting parts.

then screw / bolt got loose during operation and drop or dislodged.
velly common thing la.. even for car parts some bolts and nuts will cum loose after shaking too much
to catch this uslally is to turn on engine then look in engine bay / check before driving .
if can watch daily see anything wrong before driving off then good, otherwise check weekly or monthly will catch the problem.

moi agree with chiu la.
lecently moi changed a part in moi car then screw back everything naisly.
then everything good.
then one day moi open the bonnet while engine running then saw the part moi replaced vibrating viloently..
then moi take closer look the 2 bolts securing it missing .. moi quickly turn off engine and found replacement bolt and tighten .. next taim need thread locker.
anyway lucky moi found out otherwise the part drop off then got big issue

Looks serious. Now we need to wait for FTs from less developed country to fix it. Frankly, moi think even if it got fixed, it's just a temporary & another breakdown can be expected.

Commuters mainly care about how quickly the issue is resolved and when train service will resume.
